Medicare ServicesMedicare Skilled Services is a new program to Family Care Network that provides skilled nursing and therapy services to homebound patients. Family Care Network in coordination with Family Home Health Services, a Medicare certified and CHAP accredited home care agency, is now able to provide skilled intermittent and part-time care to Medicare eligible individuals to promote independence in the comfort of their own homes. These services are designed to provide home health rehabilitation staff on an "as needed" basis. All home health rehabilitation visits are subject to applicable requirements of the federal Medicare Program, and the clinical policies and procedures of Family Care Network and Family Home Health Services. Treatment is provided on a one-on-one basis in the comfort and privacy of your home. Medicare Services can be provided for the following reasons:
To determine medical care needs a comprehensive assessment (OASIS) is conducted by a Registered Nurse or Physical Therapist. A plan of care is developed and services are provided by the following professional staff under the supervision of Registered Nurse:
Family Care Network's Skilled Care and Therapy Services are available to Medicare eligible individuals residing in Wayne, Oakland, and Macomb Counties.
