Medicare Services

Medicare Skilled Services is a new program to Family Care Network that provides skilled nursing and therapy services to homebound patients.  Family Care Network in coordination with Family Home Health Services, a Medicare certified and CHAP accredited home care agency, is now able to provide skilled intermittent and part-time care to Medicare eligible individuals to promote independence in the comfort of their own homes.

These services are designed to provide home health rehabilitation staff on an "as needed" basis.  All home health rehabilitation visits are subject to applicable requirements of the federal Medicare Program, and the clinical policies and procedures of Family Care Network and Family Home Health Services.  Treatment is provided on a one-on-one basis in the comfort and privacy of your home.

Medicare Services can be provided for the following reasons:

  • New or worsening of a health condition
  • New or changed medication
  • Teaching of treatments such as injections, blood glucose monitoring, wound care, ostomy care and other skilled procedures
  • Disturbance in gait and mobility
  • Fall Prevention
  • Evaluation of home environment to assess need for assistive devises to improve mobility, independence and safety

To determine medical care needs a comprehensive assessment (OASIS) is conducted by a Registered Nurse or Physical Therapist.  A plan of care is developed and services are provided by the following professional staff under the supervision of Registered Nurse:

  • Licensed Practical Nurse to assist with wound care, injections, blood glucose monitoring, ostomy care and other skilled procedures. LPN will also provide training on the disease processes and implications, medication administration, side effects and implications.
  • Physical Therapy to assist in restoring functional mobility, improving ability to perform activities of daily living, increasing strength and independence in the home and community. Physical Therapist also assists in the safe and proper use of equipment such as a cane, walker, wheelchair or orthotic-prosthetic devices.
  • Occupational Therapy to evaluate your home and arrange for needed assistive devices to increase your mobility, safety and independence in your home.
  • Medical Social Worker to assess social and emotional factors, identify eligibility for other services and benefits, referrals to other needed services, crisis intervention and short-term therapy.
  • Certified Nursing Assistant provides skilled care with bathing, and other personal care needs.
